The Challenge: Barriers to Health in Our Community
Gwinnett County is the most diverse county in the Southeastern United States. While our diversity is a tremendous strength, it also comes with complex challenges—especially when it comes to healthcare.

When people don’t know where to go for help, they often wait too long to seek it. We’re working to change that.
Programs and Initiatives
Gwinnett Cares
Through collaboration with local nonprofits, we’ve streamlined communication to reach Gwinnett’s population of over a million people. We connect our community to essential resources and services, from healthcare and housing to multicultural assistance and employment.
Gwinnett Health Finder
With our Gwinnett Health Finder, powered by the Gwinnett Coalition in partnership with GNR Public Health, Gwinnett’s uninsured and underinsured population can find and access quality, affordable healthcare.
Our Solution: A Coalition-Led Health Literacy Education and Resource Model for Equity
Building Health Literacy from the Ground Up
We help residents take charge of their health by building strong health literacy skills in both English and Spanish. Our programs give individuals the confidence and practical knowledge to make informed decisions about their physical, mental, and social well-being.
Whole-Person Wellness Education
Our evidence-informed health literacy workshops address the complete health triangle: physical, mental, and social health. Participants learn how each area connects to daily life, gaining tools and insights that support long-term wellness.
Bilingual and Culturally Responsive Programs
Our bilingual health workshops are led by trusted facilitators from the communities we serve. Materials are inclusive, culturally relevant, and designed to meet participants where they are, ensuring accessibility for everyone.
Interactive and Practical Learning
We believe in learning by doing. Participants engage in interactive health activities, guided discussions, and personalized exercises that build confidence and real-world skills. These aren’t lectures; they’re collaborative conversations that empower individuals to take ownership of their health.
Access: Connecting People to Preventive Care and Resources
Onsite Care Delivery
Knowledge is just the start. We bridge education with immediate access to preventive healthcare and community resources, ensuring residents can take action for their well-being on the spot. Through strong partnerships with local health organizations, our workshops provide free health screenings, vaccinations, and direct support services to meet people where they are.

Blood pressure, glucose, and STI screenings
Flu and COVID-19 vaccinations
Nutrition education and healthy food boxes
Preventive care and chronic disease management materials
Coalition-Powered Coordination
Behind every event is a network of coalition partners working together to deliver a seamless, compassionate experience. We coordinate healthcare, social service, and community organizations to ensure residents receive consistent, connected, and culturally informed support.
Community-Centered Delivery: Meeting People Where They Are
We don’t ask people to come to us—we go to them.
By working with trusted local organizations, we embed bilingual, culturally relevant health literacy and care into the places where people already live, work, worship, and gather.

The state of Georgia has one of the highest maternal mortality rates in the United States, an alarming statistic that deserves a priority response. The Gwinnett Coalition’s Infant & Maternal Mortality (IMM) Action Team was established in 2019 to address this issue. The IMM Action Team aims to improve the health and well-being of mothers in Gwinnett County by increasing access to quality prenatal and postnatal care, providing education and prevention services to healthcare providers and the community, promoting maternal mental health, supporting the Safe to Sleep Campaign and introducing evidence-based group prenatal care programs.
